Contribution up to CZK 400 once per year for a dermatoscopic skin mole examination, available to all policyholders.

Contribution up to CZK 800 once every two years: for women from age 30 for an ultrasound or mammography examination (women from age 45 have the preventive examination covered by public health insurance every other year; the contribution is meant for the in-between years), and for men from age 30 for an ultrasound examination.

Contribution up to CZK 300 once per year for men from age 40 for a preventive PSA blood test or a prostate ultrasound examination, if not covered by public health insurance.

Contribution up to CZK 300 once per year, available to all policyholders for the purchase of a self-sample test kit for colorectal cancer prevention, or for policyholders under age 45 for a screening examination performed at a healthcare facility (from age 45 the examination is covered by public health insurance).

Contribution up to CZK 1,500 once per year for policyholders from age 15 until they turn 18 for vaccination against human papillomavirus (HPV) as recommended by a physician. (At ages 11–14 the vaccination is covered by public health insurance.) The contribution cannot be used for the co-payment of a partially covered vaccine.

CZK 200 bonus for policyholders aged 45–75. Conditions: a physician-evaluated fecal occult blood test in the past 24 months or a colonoscopy in the past 10 years. Bonuses can be claimed electronically (portal/app) or in person at a contact point (not by paper form).

CZK 100 bonus for women from age 45. Condition: a completed mammography examination in the past 24 months. Bonuses can be claimed electronically (portal/app) or in person at a contact point (not by paper form).

CZK 100 bonus for men aged 50–70. Condition: active participation in an early-detection prostate cancer screening program within the past 4 years at most (participation must not have been terminated early by the policyholder). Bonuses can be claimed electronically (portal/app) or in person at a contact point (not by paper form).
